Choosing A Service Provider
You Have the Right to Choose
Azelea Hospice offers a supportive care, medical management, and comfort to patients and families. Our services may include:
Drugs, medical supplies, durable medical equipment, palliative therapies, wound care, when needed for pain and symptom management.
Bereavement services continue for the family and friends for 13 months.
Interview Each Provider
Questions to Asks before Choosing Hospice Service
Who will meet with me? (Will it be a member of your care team or a sales person?)
Can I call past families for references?
Is there an “on call team” or just one nurse?
Will they treat symptoms from head to toe?
Are they owned locally?
Can I speak with the owner?.
Levels of Hospice Care
1. Routine Home care
Basic care available wherever the patient calls home (residence, nursing home or assisted living facilities).
2.Continuous Care
Medical management in the home for up to 24 hours a day when medically appropriate.
3.Short-Term Inpatient Care
When medical needs cannot be managed in the home.
4.Respite Care
Allows the patient a brief inpatient admission to provide caregivers an opportunity to take some time off or “respite”.
